This document includes a description of your client, the facts surrounding the loss, your client's medical history, injuries, treatment, wage loss, the strengths and weaknesses of your case and any missing information the insurance company may require prior to extending an offer of settlement.

MEDICAL BILL SUMMARY. This is an itemization of the medical bills by provider and date.

Send your client's medical records to us at medicalrecordsummaryservice@yahoo.com through a secure file transfer service (i.e., Dropbox).

Include the following information with your request: Your client's name, date of loss, a brief description of the incident, and your desired completion date.

Once we receive the documents we will respond with an acknowledgement of the

assignment, a fee estimate and an estimated completion date.

When our review is complete, the documents will be sent to you via e-mail along with our

invoice, which can be paid via credit card, PayPal, Venmo or check. Larger cases may require

a deposit.
